The goal of the therapy is
1/.Suppression of testosterone
2/.Augmentation of femininazing hormones such as estrogenes and progesterone.
Additionally anti-androgens such as spironolactone and finasteride can be used.
The following changes are expected after male to female hormone therapy is initiated: breast enlargement, increased body fat, decreased growth of body and facial hair, smaller testicular size and lowered erectile function. The extent of these changes and the time duration for the desirable effects is different for individual patients and may take up to 18 to 24 months to happen. Use of anti-androgenic therapy as an addition may help to achieve maximum results.

The best assessment of hormone efficacy is clinical response: Is a patient developing a feminized body while minimizing masculine characteristics, consistent with that patient’s gender goals?
Periodic assesment of hormone levels through laboratory testing is a guiding tool. Testosterone is usually being suppressed to the upper limits of a normal female range.
Estrogen is elevated to the range of a premenopausal woman.
